Harry Sotnick House is a busy 76 bed nursing home, currently divided into a 46 bedded nursing unit on the ground floor and currently a 30 bedded Discharge to assess unit (D2A) on the first floor. and is rated as Good by the Care Quality Commission. The nursing unit delivers a broad range of healthcare services to older people with mental and physical health needs, specialising in long term dementia care.  The D2A unit works closely with our NHS partners to enable a prompt discharge from hospital for further assessment of their medical and social needs. We work closely with the in-house assessment team, including social workers, occupational therapist and physiotherapists to enable our clients to be discharged to the most appropriate setting. The D2A unit is a fast-paced unit with admissions and discharges on a daily basis. A well-resourced multi-disciplinary team is available who are fully supportive in providing the highest quality care to our services users

These vacancies are being advertised on both the Discharge to Assess Unit and the Nursing Home Unit and there is an expectation that Nurses employed at Harry Sotnick House may be asked to work across both units.

The main responsibilities of this role include:

  • Promoting the highest standards of clinical nursing throughout the nursing home
  • Overseeing the day-to-day care of patients and the administration of prescribed medication
  • Planning, implementing and supervising the provision of quality care
  • Maintaining accurate clinical records
  • Reporting any clinical concerns and requesting professional visits when necessary
  • Ensuring the care, welfare and safety of all residents is maintained at the highest possible standard.
  • Creating and updating individualised care plans.
  • Using a person-centred approach to ensure every resident’s needs are understood and met with the greatest privacy, dignity, and respect.
  • Demonstrate effective communication skills with residents, their families and friends, providing information and advice where appropriate.
  • Managing shifts and handovers to ensure continuity of care

The main requirements for this role are:

  • Must be a registered nurse (current PIN number with NMC ideally)
  • The ability to communicate clearly and concisely, along with excellent listening skills
  • The ability to always maintain a optimistic attitude
  • Active and hands-on
  • Thoughtful, caring and self-confident
  • Able to prioritise and work under pressure

A proven track record of working with older people with complex care requirements, ideally within a hospital or clinical setting is an advantage, as is experience in wound care, palliative care, catheterisation, phlebotomy, and the deteriorating person.
